One of the main aims of the service is to act as a steppingstone or bridge between hospital and independent living in the community. We will achieve this by:
Collaboration between all stakeholders involved in the person’s support and care is imperative to the individual to achieve their own goals and success
The PCCP is a live plan, it changes as the needs of the person changes
The aim of this programme is to integrate an individual into “normal” community life once again, a life where the person is and feels valued and participates in their own community
